HOW TO BOOK:

Please follow these instructions to reserve your appointment:

• Go to this website: https://hawkesbury.inputhealth.com

• Click on the "Patient" icon.

• Since you are a Hawkesbury Family Health Organization patient, click "Yes."

• Enter your information (Last name, Date of birth, Telephone number, and Email). For ''IDENTIFICATION,'' choose the province of your health card and enter your health card number.

• You will receive a one-time passcode to your email. You need this passcode to be able to log in.

• Once you are logged in, select "Ruby" as the location.

• Select the type of appointment that you need with your primary care physician.

• Select the date and time of your appointment. 

• Verify your personal information and enter your email address to receive your appointment confirmation. For "NOTIFICATION METHOD," select "Email." We do not communicate via text message.

• Vous recevrez un code à usage unique sur votre courriel. Vous aurez besoin de ce code pour vous connecter.

• Une fois connecté, sélectionnez la clinique "Ruby" comme lieu.

• Choisissez le type de rendez-vous dont vous avez besoin avec votre médecin de famille.

• Sélectionnez la date et l'heure de votre rendez-vous.

• Vérifiez vos informations personnelles et saisissez votre adresse électronique pour recevoir la confirmation de votre rendez-vous. Pour le "MODE DE NOTIFICATION", choisissez "Courriel". Nous ne communiquons pas par message texte.
